You acknowledge that standard email/web forms may not be 100% secure; for sensitive medical inquiries, we recommend using our secure CharmHealth Patient Portal.

5. NOTICE OF PRIVACY PRACTICES (NPP)

For a full description of your rights regarding your medical records, including your right to access and amend your PHI. Please send your request to medical records at the follow email: medicalrecords@raynorcareclinic.com. You can also request the records by visiting the clinic in person.
